Physical Exam and Birth Certificate Requirements

Each athlete must have a recent physical exam (within the last 6 months) in order to participate in the program. Physical exams may be obtained by your personal physician, Walk-In Clinic or First Care. Physical forms must be turned in prior to or on the first day of practice.
