Physical exams may be obtained by your personal physician, Walk-In Clinic or First Care. Physical forms must be turned in prior to or on the first day of practice.

A photocopy of your child’s birth certificate should be provided at the time of registration or by the first day of practice.

ANY ATHLETE MISSING ANY OF THE ABOVE INFORMATION WILL NOT BE PERMITTED TO PRACTICE OR CHEER AT ANY GAME UNTIL ALL INFORMATION HAS BEEN RECEIVED. THIS IS A REQUIREMENT FOR OUR INSURANCE – NO EXCEPTIONS!
